For your initial appointment please download our Client and Patient Information form and fill it out as completely as you can. Bring this, as long with any previous records you may have, along to your appointment.
- Due to high demand, if an appointment is not confirmed within 24 hours of the appointment time, it will unfortunately be canceled.
If you were referred by an existing client remember to communicate this with our staff as you are scheduling or at the initial appointment.
